With the German National Cohort (NAKO), we want to learn as much as possible about how developing diseases are linked to lifestyle, physical activity or dietary behaviour, for example. Our questionnaires on these topics are an important part of the study and your support in this is a key component of our scientific research.
Here you will find an overview with links to all online questionnaires and the online application PIA. You will receive your access data for the individual questionnaires during the second re-examination at your study centre.

PIA app: over 25 questionnaires on lifestyle habits and circumstances
We have replaced the touchscreen survey with the digital application PIA, which we use for a large proportion of the questionnaires in the German National Cohort (NAKO). With the help of PIA, we collect information on topics such as health-related quality of life, physical activity, tobacco and alcohol consumption, family planning and pregnancy as well as housing situation. If you do not want to or cannot answer individual questions in PIA, you have the option of skipping them without answering them.
Everyday activities of the past 24 hours
This questionnaire is used to track everyday activities over the past 24 hours. The data can be used to investigate the type of physical activity, what purpose it fulfils and which population groups are sufficiently physically active and whether the level of activity is related to possible illnesses and, for example, quality of life.
Important information
- Once you have started the questionnaire, you will need to finish it on the same day. You will not be able to access it again on another day!
- Please complete this questionnaire two days after your visit to your study centre. Please do not answer the questionnaire earlier, so that the description of your activities does not refer to your day at the study centre.
- If you have received an accelerometer from us, please complete this questionnaire in such a way that you report on a day on which you wore the accelerometer.
